(Consumer Information Disclosure)
The Policy, adopted on October 20, 2015, reads as follows: In accordance with Illinois Public Act 099-0278, Illinois Eastern Community Colleges will ensure that, at or near the time that an incoming student enrolls, he or she is provided the opportunity to authorize in writing the disclosure of certain private mental health information to a designated person. The associated Procedure 500.12 provides additional instruction and the disclosure form.
